Students

Typical Student Forms:

Degree Plan

Use this form for a new degree plan or to make changes to an existing degree plan.  This form must be on file with the HSC Registrar's Office.

 

Committee Membership Approval

Use this form to secure committee membership.  It is preferred that this form be on file with the HSC Registrar's Office.

 

Proposal for Thesis/Dissertation

Use this form to secure committee approval for a thesis/dissertation topic.  This form must be on file with the HSC Registrar's Office.

 

Oral Preliminary Exam Evaluation

Use this form to verify that a candidate has fulfilled the requirements of the oral portion of their preliminary examination.

 

Written Preliminary Exam Evaluation

Use this form to verify that a candidate has fulfilled the requirements of the written portion of their preliminary examination.

 

Final Defense Request and Announcement

Use this form to request permission for and to announce a candidate's defense and final examination.  It is preferred that this form be on file with the HSC Registrar's Office.

 

Oral Defense Evaluation

Use this form to verify the outcome of the oral portion of a student's thesis/dissertation.  This form must be on file with the HSC Registrar's Office.

 

Written Defense Evaluation

Use this form to verify the outcome of the written portion of a student's thesis/dissertation.  This form must be on file with the HSC Registrar's Office.

 

Transfer or Non-HSC Student Forms:

Non-Degree Application (TAMU)

Use this form if you are currently a Texas A&M University student who wishes to take courses at the Health Science Center.  This form (if applicable) must be on file with the HSC Registrar's Office.

 

Transfer Work/Advanced Standing Request

Use this form if you have work that you wish to transfer toward a HSC SGS degree.  This fom (if applicable) must be on file with the HSC Registrar's Office.

Ongoing Research Assessment

Use this form to help assess a student's progress regarding their research.

 

Student Seminar Evaluation

Use this form to assess a student's performance in a seminar.

Faculty

Request for Appointment to Graduate Faculty
Use this form to request appointment of a faculty member to the SGS faculty.  Listed below are some of the rules for submitting an appointment. 

 

Please refer to the full Guidelines for Graduate Faculty Membership for information prior to submitting the appointment paperwork.

 

Nomination for membership on the Graduate Faculty must be initiated by the head of the appropriate academic department/center or program, submitted through the associate dean/director for graduate education at each of the HSC components, and forwarded to the Office of Graduate Studies.  Appointment to the Graduate Faculty is made by the Dean of the Graduate School upon the recommendation of the Graduate Program Council.

     Nomination Guidelines
     Each nomination packet must include the following:

1. Letter from candidate's department/center head or the graduate program director through the associate dean/director describing briefly: a) the background of the nominee and b) the proposed role of the nominee in the graduate program.

2. Completed SGS form: Request for Appointment to Graduate Faculty.

3. Complete curriculum vitae (biosketch is not acceptable).
