Student Forms
Below are the various forms to be used throughout a student's program of study at the School of Graduate Studies. These forms have been developed and approved by the Graduate Program Council and are to be used for SGS students, regardless of their program or campus location.
Student Progress/Program Completion Forms:
- Use this form to outline a new degree plan or to make changes to an existing degree plan. This form must be on file with the HSC Registrar's Office.
- Use this form to secure committee membership. It is preferred that this form be on file with the HSC Registrar's Office.
Advisory Committee Meeting Evaluation
- Use this form to evaluate a student's progress during their yearly Advisory Committee Meeting. This form must be submitted to your location-based Office of Graduate Studies on an annual basis. Students will be blocked from registration until this form is completed each year.
Proposal for Thesis/Dissertation
- Use this form to secure committee approval for a thesis/dissertation topic. This form must be on file with the HSC Registrar's Office.
Oral Preliminary Exam Evaluation
- Use this form to verify that a candidate has fulfilled the requirements of the oral portion of their preliminary exam.
Written Preliminary Exam Evaluation
- Use this form to verify that a candidate has fulfilled the requirements of the written portion of their preliminary exam.
Final Defense Request and Announcement
- Use this form to request permission for and to announce a defense and final examination. It is preferred that this form be on file with the HSC Registrar's Office.
- Use this form to verify the outcome of the oral portion of a student's thesis/dissertation. This form must be on file with the HSC Registrar's Office.
- Use this form to verify the outcome of the written portion of a student's thesis/dissertation. This form must be on file with the HSC Registrar's Office.
Miscellaneous Forms:
- Use this form to file a formal complaint regarding any issue for which there is not already a policy or procedure in place. For additional information, refer to the Student Miscellaneous Complaint webpage.
Student Travel Grant Application
- Use this form to apply for a Student Travel Grant worth up to $500. For additional information, refer to the SGS Career Development Grants webpage.
Transfer/Non-HSC Student Forms:
Non-Degree Application (TAMU Students)
- Use this form if you are currently a Texas A&M University student who wishes to take courses offered at the Health Science Center. This form (if applicable) must be on file with the HSC Registrar's Office.
Transfer Work/Advanced Standing Request
- Use this form if you have work that you wish to transfer toward an SGS degree. This form (if applicable) must be on file with the HSC Registrar's Office.
